- a subject line or header that says "subject access request";
- the date you're making the request;
- your name (and any other names where relevant, eg your name before you were married);
- your email address, home address and phone number;

An individual can make a SAR verbally or in writing, including on social media. A request is valid if it is clear that the individual is asking for their own personal data. An individual does not need to use a specific form of words, refer to legislation or direct the request to a specific contact.

As well as the requester's personal data, you need to send your privacy information. They have a right to know why you hold their data, how you got it, how long you're planning on keeping it, who you share it with, and how they can ask for it to be changed (such as updating their address) or deleted.
